Packet sections to prepare
- Provider name, address, and tax ID note if required by your administrator.
- Dependent or child name for each receipt or statement.
- Service start and end dates, not only payment date.
- Amount billed or paid for eligible dependent care.
- Provider signature, certification, or itemized statement when requested.
- Submitted month status to reduce duplicate claims.
- PDF packet for upload or review, plus CSV annual log for your own records.
Workflow before submission
- Group receipts by provider, dependent, and month.
- Review each receipt for missing service dates, amount, provider details, dependent name, and signature or itemization.
- Mark months already submitted so they do not get accidentally repeated.
- Export a PDF packet for your own claim workflow.
- Keep a CSV annual log for year-end review and follow-up.
- Submit only through your administrator's official portal or process.
